Almost all problems due to image uploads are from strange filenames.
Lets all help the system, with some basic rules to follow:
1) I call it the rule of children. Dont use a character in a filename that you couldnt name a small child after. While you could certainly name him "A" you couldnt name her " ". You could also name her "Z", but definatly not " ' ". For example:
Good: APicOfMyself.jpg
Bad: A picture of me painting Aharon's house!~!@@!!!.jpg
PS - Putting spaces in filenames is not a cool thing to do. It breaks just about every rule of filesystem design there is. Why windows allows it is beyond me. It also doesnt make you look intelligent.
2) Dont change the extenion of a filename.
Good: APicofMyself.jpg
Bad: APicofMyself.here
